Our office now has forms available online through our patient portal! Give us a call at (360) 676-4488 and we will email you a link to join our portal where you can fill out forms, view your upcoming appointments, and make payments.

  • For patients new to the office or who haven't been in in over three years, please fill out the New Patient Paperwork.

  • For new or existing patients who are coming in for treatment following an injury sustained at work or who were involved in a car accident please fill out the MVA or Work Injury paperwork.

  • If you are an established patient who hasn't been in to the office in the last 10+ months, has a change of address or insurance, or has experienced new health issues please fill out the Existing Patient Update Form.

The Chiropractic Office is in network with most major insurance companies.  We also handle work injury and motor vehicle accident claims. If you have any questions regarding your insurance provider’s coverage of chiropractic care, please contact our office for assistance.
